Evidence suggests that housing instability and lack of access to primary and mental health services lead to high utilization of emergency departments and hospitalization. Chronic conditions such as cancer and heart disease fueled by tobacco use in approximately three-quarters of the homeless population represent significant causes of death. Infections, injuries, and other acute conditions fester, and communicable diseases such as tuberculosis, AIDS, and viral hepatitis readily spread. Patient-centered and community health models can mitigate poor health outcomes for this population. Learn about a nurse practitioner-led health model utilizing evidence-based practices for curbside care to provide access to primary care, mental health, and social services.
